The mission of the Community Health Network of West Virginia is to facilitate health improvement and excellence in the delivery of health care services by our members through appropriate use of technology, continuous training and integration of operational resources.
In pursuit of our mission, we believe the following goals and objectives are essential and should guide us in our activities:
Create an integrated community health care system that assures excellence in the delivery of primary and preventive health services and addresses the chronic conditions of the patients of our member health care organizations.
Develop and deploy technology applications that encourage the appropriate capture, use and exchange of health information to improve health outcomes.
Facilitate continuous improvement through focused training and educational efforts and coordination and integration of administrative functions to promote greater effectiveness and efficiency of health care delivery.
Develop and strengthen strategic relationships that promote these objectives.
